Genomic investigation of multispecies and multivariant blaNDM outbreak reveals key role of horizontal plasmid transmission

Objectives:
New Delhi metallo-β-lactamases (NDMs) are major contributors to the spread of carbapenem resistance globally. In Australia, NDMs were previously associated with international travel, but from 2019 we noted increasing incidence of NDM-positive clinical isolates. We investigated the clinical and genomic epidemiology of NDM carriage at a tertiary-care Australian hospital from 2016 to 2021.
Methods:We identified 49 patients with 84 NDM-carrying isolates in an institutional database, and we collected clinical data from electronic medical record. Short- and long-read whole genome sequencing was performed on all isolates. Completed genome assemblies were used to assess the genetic setting of blaNDM genes and to compare NDM plasmids.
Results:Of 49 patients, 38 (78%) were identified in 2019–2021 and only 11 (29%) of 38 reported prior travel, compared with 9 (82%) of 11 in 2016–2018 (P = .037). In patients with NDM infection, the crude 7-day mortality rate was 0% and the 30-day mortality rate was 14% (2 of 14 patients). NDMs were noted in 41 bacterial strains (ie, species and sequence type combinations). Across 13 plasmid groups, 4 NDM variants were detected: blaNDM-1, blaNDM-4, blaNDM-5, and blaNDM-7. We noted a change from a diverse NDM plasmid repertoire in 2016–2018 to the emergence of conserved blaNDM-1 IncN and blaNDM-7 IncX3 epidemic plasmids, with interstrain spread in 2019–2021. These plasmids were noted in 19 (50%) of 38 patients and 35 (51%) of 68 genomes in 2019–2021.
Conclusions:Increased NDM case numbers were due to local circulation of 2 epidemic plasmids with extensive interstrain transfer. Our findings underscore the challenges of outbreak detection when horizontal transmission of plasmids is the primary mode of spread.

Abstract
BackgroundAltered expression of the complement component C4A gene is a known risk factor for schizophrenia. Further, predicted brain C4A expression has also been associated with memory function highlighting that altered C4A expression in the brain may be relevant for cognitive and behavioral traits.
MethodsWe obtained genetic information and performance measures on seven cognitive tasks for up to 329 773 individuals from the UK Biobank, as well as brain imaging data for a subset of 33 003 participants. Direct genotypes for variants (n = 3213) within the major histocompatibility complex region were used to impute C4 structural variation, from which predicted expression of the C4A and C4B genes in human brain tissue were predicted. We investigated if predicted brain C4A or C4B expression were associated with cognitive performance and brain imaging measures using linear regression analyses.
ResultsWe identified significant negative associations between predicted C4A expression and performance on select cognitive tests, and significant associations with MRI-based cortical thickness and surface area in select regions. Finally, we observed significant inconsistent partial mediation of the effects of predicted C4A expression on cognitive performance, by specific brain structure measures.
ConclusionsThese results demonstrate that the C4 risk locus is associated with the central endophenotypes of cognitive performance and brain morphology, even when considered independently of other genetic risk factors and in individuals without mental or neurological disorders.

Summary
Executive Summary
To achieve higher renewable energy (RE) shares than the low levels typically found in present energy supply systems will require additional integration efforts starting now and continuing over the longer term. These include improved understanding of the RE resource characteristics and availability, investments in enabling infrastructure and research, development and demonstrations (RD&D), modifications to institutional and governance frameworks, innovative thinking, attention to social aspects, markets and planning, and capacity building in anticipation of RE growth.
In many countries, sufficient RE resources are available for system integration to meet a major share of energy demands, either by direct input to end-use sectors or indirectly through present and future energy supply systems and energy carriers, whether for large or small communities in Organisation for Economic Co-operation and Development (OECD) or non-OECD countries. At the same time, the characteristics of many RE resources that distinguish them from fossil fuels and nuclear systems include their natural unpredictability and variability over time scales ranging from seconds to years. These can constrain the ease of integration and result in additional system costs, particularly when reaching higher RE shares of electricity, heat or gaseous and liquid fuels.
Existing energy infrastructure, markets and other institutional arrangements may need adapting, but there are few, if any, technical limits to the planned system integration of RE technologies across the very broad range of present energy supply systems worldwide, though other barriers (e.g., economic barriers) may exist. Improved overall system efficiency and higher RE shares can be achieved by the increased integration of a portfolio of RE resources and technologies.

The nature of the dispersion of a clay platelet in a resin composite will play an important role in the process of enhancement of the physical properties of that material. This paper describes studies of ultrasonically and mechanically mixed dispersion of various clays in methyl methacrylate (MMA) and reports the effects of organic surface modification on the rheology, suppression of settlement of filled systems, cure characteristics and final mechanical properties.

Background
Suicide reduction is government strategy in many countries. We need to quantify the connection between non-fatal self-poisoning and eventual suicide.
AimsTo determine mortality after an episode of self-poisoning and to identify predictors of death by any cause or by suicide.
MethodA retrospective single-group cohort study was undertaken with 976 consecutive patients attending a large accident and emergency unit in 1985–1986 after non-fatal self-poisoning. Information about deaths was determined from the Office for National Statistics.
ResultsOf the original patients, 94% were traced 16 years later; 17% had died, 3.5% by probable suicide. Subsequent suicide was related to numerous factors evident at the time of the episode of self-poisoning but, when examined for their independent effects, only the severity of the self-poisoning episode and relevant previous history seemed important.
ConclusionsPatients attending a general hospital after self-poisoning all require good basic assessment and care responsive to their needs. Attempts to reduce the huge excess of suicide subsequent to self-harm are not likely to achieve much if they are based on the identification of subgroups through ‘risk assessment’
Biostratigraphy of late Llandovery (Telychian) and Wenlock turbiditic sequences in the SW Southern Uplands, Scotland

In the SW part of the Southern Uplands of Scotland the relatively thin Moffat Shale Group (late Ordovician–early Silurian) is succeeded by a thick development of Silurian greywackes, of variable turbiditic facies. This includes late Llandovery (Telychian) quartzose greywackes with interbedded thin graptolitic shales of the turriculatus and crispus biozones, in the upper part of the Gala Group, a sequencewhich is laterally equivalent to the basal part of the Hawick Group. The age of the finer-grained calcareous Hawick Group, which here includes 1. the Ross Formation, ranges from late Llandovery (turriculatus Biozone) to early Wenlock (riccartonensis Biozone). The Riccarton Group, which contains thick units of thinly-bedded siltstones and mudstones, is of Wenlock age (riccartonensis to lundgreni biozones). Within this sequence, all the biozones of the standard graptolite zonal scheme have been recognised in the area, with the exception of the crenulata Biozone of the late Llandovery (Telychian Stage) and the murchisoni and ellesae biozones of the Wenlock (Sheinwoodian Stage). Details of the graptolite biostratigraphy areclosely comparable with those of the markedly thinner sequences of northern England. Acritarchs occur throughout the sequence but are most numerous and best preserved in the Gala Group. Poorly preserved chitinozoa and spores are also present, the former occurring sporadically throughout the succession but the latter become common only in the Riccarton Group.

Recent evidence supports the notion that capital markets are substantially integrated in a return/systematic risk sense [1, 12]. Other studies show that investors can reduce the total level of risk borne without reducing expected returns by holding an internationally diversified portfolio [8, 9, 17]. Together these studies imply that the various economies mirrored by financial markets are not perfectly integrated. Some assert, however, that because of controls on capital flows, differential trading costs, different tax structures, and a number of other factors, markets are imperfectly (albeit substantially) integrated. Hence investors may not actually be diversifying internationally and thus forego advantages which would accrue to them if they were willing to hold foreign security issues.
